Artificial Sweetener Xylitol Linked to Increased Risk of Cardiovascular Events

A recent study has raised concerns about the safety of xylitol, a widely used artificial sweetener. Researchers have found that higher levels of xylitol in the bloodstream may be associated with an increased risk of serious cardiovascular events, including heart attacks and strokes.

Presented at the European Society of Cardiology Congress 2026 in Munich, Germany, the study’s findings have not yet been published in a peer-reviewed journal. Marco Witkowski, a researcher from Charité University Hospital in Berlin, emphasized the need for further investigation into the long-term cardiovascular safety of xylitol, noting that many consumers perceive artificial sweeteners as healthier alternatives to sugar.

Witkowski highlighted that previous studies indicated xylitol could enhance platelet activity in the blood, which may lead to clotting. His new research expands on this by examining how xylitol affects cardiovascular risk among the general population. The study analyzed data from over 17,000 participants across two cohorts in Canada and the United Kingdom.

The results indicated that individuals with the highest blood levels of xylitol faced a significantly elevated risk of experiencing major cardiovascular issues. In the Canadian cohort, over a six-year follow-up, those with elevated xylitol levels had a 57% higher risk of adverse cardiovascular events compared to those with the lowest levels. Similarly, in the U.K. cohort, a 30-year follow-up revealed an 18% increased risk for participants with high xylitol concentrations.

These findings suggest a dose-dependent relationship, where higher xylitol levels correlate with greater cardiovascular risk, even when controlling for other known risk factors such as hypertension, smoking, age, and diabetes.

The implications of this research contribute to an ongoing dialogue about the health impacts of artificial sweeteners. Cheng-Han Chen, a cardiologist at MemorialCare Saddleback Medical Center, noted that while artificial sweeteners have been regarded as healthier alternatives to sugar, emerging evidence suggests they may carry unintended health risks, particularly concerning heart health. However, he cautioned that more research is needed to establish a direct cause-and-effect relationship.

Xylitol is a sugar alcohol that occurs naturally in small amounts in the body and is commonly added to various food and dental products as a sweetener. As its use becomes more prevalent, the potential long-term health effects remain uncertain.

Experts like Dana Hunnes, a senior dietitian at UCLA Health, have voiced concerns about the increasing reliance on artificial sweeteners. She suggests that consuming small amounts of real sugar might be preferable to using artificial substitutes, especially as research continues to unveil possible health detriments associated with these sweeteners.

In light of these findings, the focus on heart health should encompass a broader dietary approach. Dr. Eleanor Levin, a cardiologist at Stanford University, emphasizes that lifestyle changes, such as adopting a balanced diet rich in vegetables and legumes, should take precedence over merely eliminating artificial sweeteners.
